Harpreet Cheema, system senior vice president of Payer Strategy and Analytics of CommonSpirit Health and representatives from Innovaccer will give the best practices for establishing a clinically integrated network as the first step towards population health management in, “Optimizing Value: Best Practices to Succeed in Alternative Payment Models,” at HIMSS25.
The talk gives a strategic focus on how physicians can be incentivized for care outside of the fee-for-service model.
Cheema will be joined by Abhinav Shashank, CEO and cofounder of Innovaccer and Hemant Gupta, associate vice president of Customer Success at Innovaccer, a company that Cheema said acts as a facilitator and helps with predictive analytics.
As healthcare costs rise, value-based care is the silver lining, he said, and the only way to address the cost of care.
CommonSpirit Health is a large health system headquartered in Chicago, that includes Dignity Health and Catholic Health Initiatives. It has made large investments in the infrastructure needed to make APM payment successful. This includes an investment in people, data warehouses, analytics on the data and cloud space to ingest claims.
AI has been a good catalyst in the analytics space, Cheema said.
The investment has paid off. CommonSpirit has 2.4 million lives in accountable care across Medicare.
“We have generated a surplus for the 2.4 million lives that we manage,” Cheema said. “We have $25 billion in total cost of care revenue.”
Accountability in the alternative payment model is 50/50 shared, Cheema said. The healthcare dividend is split with the physician.
Patients benefit from holistic care such as wellness exams and being seen by a physician before they get sick. This keeps the risk level low and the patient is using the ER and other services less, Cheema said.
Incentive starts with a framework of five key components for a clinically integrated network to establish a robust foundation for population health management.
